this form work on any hosting server supporting PHP. You can use this form on any require domain, Check domain or hosting setting in domain Manager tool or Contact with hosting administrator.
And another question... does it only works on UNIx or LINUX?

Thanks Saleem... i was able to open it with CS3. Now just one little question... I can't seem to change the text for the drop down window or the product range? I've tried everything I know but I can't seem to get the text changed.
It's done in flash 8, try to Open in FlashCS3 or Flash 8.

make sure you also rename mailer.php_safe to mailer.php

I got your message but I don't know what type of problem you are facing. It's simple I have two files one is *.swf and other is *.php. It's better to put both files in same folder. On submitting form input texts process in php file and php file generate html email.
