Form PHP with Multiple Fields

I'm using Flash CS4 and doesn't work.. uhm.. what can i do?.. do i need flash mx?

I just open the file & control + enter.. and get 33 errors like:

The name of this class, 'TextField', conflicts with the name of another class that was loaded.

HI richmo58!!! It can be maybe because the Flash version that you have, you need Flash CS3 or Flash CS4. Sorry but i cannot save it for Flash 8.

I'm looking for this kind of file for a long time ! But I have some problems...1. Opening the file the Output is writing "undefined" 2. I have not the Dreamwave to change the Email adress.I'm working with Flash CS3 in AS2.
can you help me PLEASE to solve my problems ? I need to make a file that my customer will enter his unsers and send it to my mail.

Hi Apaiso, just open the file "php2excel_csv.php" in dreamweaver and in the line number 76 change the email address; the current one is "[email protected]". hope to be of help, and i put the answer here and not sent it to your email because maybe someone else need this info. Tell me if you have doubts

Very nice form. The comboBox is stilling the focus from the browser scrollbars when I use the mouse wheel. Anyone know how to fix that?

I could not open this with flash 8 on Mac:( I did try the link here as well. What a beautiful design. I am getting flash CS 3 soon, so I guess I can just wait for a few weeks.
