i didnt create this form but i wanted to share it as i know how much its difficult to find a working ASP mail form

Perksos, you can see the sentence "HERE PUT YOUR DOMAIN SMTP ADDRESS" in there you need to change and put your host domain smtp, for example yahoo.mail.com if you stil cannt do it then i need your domain provider name i will try to find the smtp address for you.

to add your email and smtp open the send_email.asp file with a notepad and you will find it there.

As far as I know this form is just to gather emails from people and send them to the submitter. There's only a swf file there.
